How It Works

Ride Health is a platform that connects a group of disparate stakeholders to drive smoother transitions of care: patients and their families, healthcare provider organizations, health plans, care management organizations, and medical transportation services. The Ride Health coordinator platform stands at the center of this ecosystem, enabling care coordinators and social workers to arrange transportation on behalf of their patients.

Care coordinators and social workers can manage transitions of care across the continuum using our HIPAA-compliant, web-based platform.

Our Patient Profile (or Member Profile for health plans) captures detailed information about an individual’s demographic, accessibility and communication needs and preferences. Whether a simple curb-to-curb ride is needed, or door-through-door with a bariatric wheelchair, you can configure the profile to capture all of the necessary details and ensure that the right vehicle and driver are sent.

The ride creation workflow allows coordinators to create one-way, round-trip, multi-part and recurring rides to meet the needs of any clinical use case, from an outpatient appointment to a hospital or ED discharge — and everything in between. Rides can be scheduled the same day or as far in advance as needed. Our flexible “will call” feature allows patients or coordinators to activate on demand rides whenever needed within a 24-hour time frame.

Ride success is dependent on clear and timely communication. Ride Health was built with all patients in mind, not just those with smartphones.

Based on configurations in the Patient Profile, we use SMS text messages and automated landline calls to share updates with patients, caregivers and/or clinical contacts before, during and after each ride. Ride coordinators are kept in the loop with real-time status updates in the portal and email push notifications.

After an appointment is complete, the patient can activate a ride home with the touch of a ‘1’ button. Patients who do not text can call to reach a live representative, or use our automated IVR system to navigate the experience more quickly. Since technology cannot solve all problems, we often work with organizations to distribute physical “patient passes” that help patients more easily navigate the continuum of care.

Ride Health believes in coordinating the right ride, for the right patient, at the right time. Achieving this standard requires connecting a comprehensive spectrum of medical transportation to the Ride Health platform.

Ride Health leverages direct integrations to coordinate with both on-demand ride-share services (including Uber’s vast network of drivers) and hundreds of local NEMT (non-emergency medical transportation) services which can provide wheelchair-accessible, door-to-door, door-through-door, stretcher van and non-emergency ambulance levels of service.

In addition to connecting existing resources, Ride Health is focused on helping connect supply and demand to identify market gaps and encourage suppliers to fill them — whether it means bringing high-quality door-to-door and door-through-door services to a new market, or helping build capacity among existing providers for non-emergency psychiatric transportation using “secure cars.”
